Recognizing and treating blunt diaphragmatic injury in pediatric thoracic trauma

Abstract
This scenario covers the traumatic thoracic injury in pediatric trauma and possible diaphragmatic rupture. Due to the pliability of the pediatric rib cage and mediastinal mobility, significant intrathoracic injury may exist in the absence of external signs of trauma. Pulmonary contusion and pneumothorax are frequently present without rib fractures. Traumatic diaphragmatic rupture occurs in about 1% of children with blunt chest trauma, with left-sided rupture being more common. Early diagnosis of diaphragmatic rupture is difficult due to often subtle in its initial clinical presentation and frequently overshadowed by the associated injuries. Delayed diagnosis until the time of visceral herniation and strangulation is associated with significant morbidity and a mortality rate as high as 40%. In this scenario, the learner could experience the assessment and treatment of blunt diaphragmatic injury.
